Here are the forms to complete and mail to the OASIS office or download and return forms as PDFs. For safety reasons, volunteers cannot have cognitive impairments.
Includes: Volunteer Intake, Confidentiality Policy, Media Release, In Case of Emergency, Volunteer Respite Consent and a current Volunteer Schedule.

Volunteer Support
As a volunteer, you will have participants become special to you, and with disease progression, participants will phase out of OASIS to their next level of care. This can be hard! OASIS offers supportive resources for volunteers.
